The Australian Health Practitioner Regulation Authority (AHPRA)
ACMHN (Australian College of Mental health Nurses)
Emma is a full member of the ACMHN. The ACMHN is the peak professional mental health nursing organisation and the recognised credentialing body for Australia's mental health nurses.
EMDRAA (Eye Movement Desensitisation Reprocessing Association of Australia)
Emma is a qualified EMDR practitioner and is a member of EMDRAA. The EMDRAA is a professional association where EMDR practitioners and researchers set standards for the clinical use of EMDR therapy.
NDIS (National Disability Insurance Scheme)
Serenity Holistic Wellness is able to provide NDIS sessions for individuals with an NDIS package that is self-managed. Serenity Holistic Wellness are unable to accept NDIA (Agency managed) clients as we are not registered with the NDIS.
ANF (Australian Nursing Federation)
Emma is a qualified full member of the ANF. The ANF was established in 1924 and provides members with full legal, professional and industrial representation. The core business of the ANF is the industrial and professional representation of nurses, midwives and assistants in nursing.

IICT (International Institute for Complementary Therapists) Code of Ethics
Emma holds further insurance with the ICCT. The ICCT is a global membership body that provides recognition, credibility and insurance for therapists in over 1100 approved modalities.
Privacy Policy

What personal data do we collect and why?
Personal information is information or an opinion that identifies an individual. Examples of Personal Information we collect includes names, addresses, email addresses and phone numbers. We collect your personal information for the primary purpose of providing our services to you, our clients. You may unsubscribe from our mailing / marketing lists at any time by contacting us in writing. When we collect personal information we will, where appropriate and possible, explain to you why we are collecting the information and how we plan to use it.

Sensitive Information
Sensitive information includes information or opinion about such things as an individual's racial or ethnic origin, religious or philosophical beliefs, criminal record or health information. Sensitive information will be used by us only:
- for the primary purpose of which it was obtained
- for a secondary purpose that is directly related to the primary purpose
- with your consent, or where required or authorised by law.

Third Parties
Where reasonable and practicable to do so, we will collect your personal information only from you. However, in some circumstances we may be provided with information by third parties. In such a case we will take reasonable steps to ensure you are made aware of the information provided to us by the third party.

Confidentiality and Disclosure of Personal Information
As part of providing support, we will need to collect and record personal information from you that is relevant to your situation, for example;
- contact details
- family history
- medical history
- social / educational history
- details of contact we have with you
Confidentiality means that whatever you discuss in sessions remains private and will not be passed on to anyone else without your permission. However, there are some exceptions to this for instance if there is concern that you or another person is at risk of harm, or when required to do so by law. Your personal information is stored in a manner that reasonably protects it from misuse and loss and from unauthorised access, modification or disclosure. When your personal information is no longer needed for the purpose of which it was obtained, we will take reasonable steps to destroy or permanently de-identify your personal information. However, most of the personal information is or will be stored in client files which will be kept by us for a minimum of 7 years. If you are communicating by email, zoom, contact forms or Facebook, we do not have full control over the transmission or storage and you agree that by participating in such forms of communication you understand and accept that there could be personal information for which we cannot be held responsible.

Access to your personal information
You may access the personal information we hold about you and to update and / or correct it, subject to certain exceptions. If you wish to access your personal information, please contact us in writing. We will not charge a fee for your access request but we may charge an administrative fee for providing a copy of your personal information.

Referral and Information form
New psychotherapy clients will be asked to complete a referral form prior to their initial assessment session. Referral forms are not required for group programs, mindfulness and meditation, retreats, or clinical supervision. Please select 'Health Practitioner Referral' if you are a health professional making a referral. Please select 'Self referral' if you are an individual wishing to refer yourself to Serenity Holistic Wellness. Alternatively, please complete the enquiry form on the 'contact' tab.

Consent Form
All new psychotherapy clients are required to complete a consent form prior to commencement of sessions with Serenity Holistic Wellness. Upon receipt of a completed referral, or enquiry with subsequent acceptance of client to Serenity Holistic Wellness, a copy of the Terms and Conditions will be provided along with the Consent form. Prior to the commencement of therapy, clients must read, sign and return the Consent form.

Terms & Conditions
Upon receipt of a completed referral, or enquiry with subsequent acceptance of client to Serenity Holistic Wellness, a copy of the Terms and Conditions will be provided. Prior to the commencement of therapy, clients must read, sign and return the Terms and Conditions form.

Payment Terms
A financial Agreement will be entered in to upon acceptance of engagement with Serenity Holistic Wellness. All clients will be asked to complete this form prior to sessions commencing. By signing this form, you are agreeing to make payment prior to the commencement of any and all sessions with Serenity Holistic Wellness. We will not be able to provide service without this form being completed. Serenity Holistic Wellness has EFTPOS facilities or alternatively a bank deposit can be made.

Emma has a current Working with Children check #2230939 and a Police Clearance.
